“Rapid Orthodontics ” is a biological-based concept and is not dependent on type of orthodontic appliance. Rapid orthodontics that is biologically inspired involves a clinical procedure that alters alveolar bone metabolism and initiates a regional acceleratory phenomenon (RAP) healing response; this intentional act renders alveolar bone less mineralized (osteopenic) which facilitates orthodontics treatment. Alveolar decortication, an outpatient surgical procedure, results in 1) shorter active treatment time, 2) enhanced scope of malocclusion treatment, 3) more robust periodontal health , and 4) increased stability of clinical outcome (less relapse) if decortication is combined with augmentation grafting. An update for 2012 is provided.

Selective alveolar decortication traumatically injures alveolar cortical bone and induces tissue generation and high tissue turnover in the precise anatomic area of desired tooth movement.The alveolar spongiosa undergoes rapid transformation (demineralizationremineralization) as the body attempts to heal from the trauma; the marked tissue turnover (osteopenia) facilitates rapid tooth movement. The patient is seen every 2 weeks, and 6 month active orthodontic treatment times are common even for severe malocclusions. Clinical outcomes research has shown better settling during retention, more stable results, and incisor alignment “relapse” is virtually absent. These facts are likely due to the high tissue turnover as well as the thicker cortical bone resulting from grafting. The several biology-based techniques used to stimulate alveolar bone RAP raises questions about which method is best. This presentation reviews the various “Rapid Orthodontics” techniques and answers questions such as: Which is the least invasive technique? Is a surgical flap really necessary? Can the alveolus be injured only on the labial? Is augmentation grafting really needed? In addition, indications for the various techniques will be reviewed relative to the type of malocclusion presenting. All contemporary biology-based rapid orthodontic techniques will be reviewed (not to be confused with logistics-based “Surgery-First” technique).
